Low back pain is a common condition in the general population. An often overlooked source of low back pain in the general population is the consequence of internal disc disruption. This condition was described by Crock in 1970 and continues to be a diagnostic and therapeutic challenge to physicians. This article presents the relevant anatomical, biochemical, and biomechanical models; clinical presentation; and diagnostic tests. It discusses nonoperative and operative treatment options. And it outlines a goal-directed rehabilitation program designed to return individuals to their previous level of functioning and to prevent further recurrences.
