A Study of Fine and Manual Hand Dexterity in People with Parkinson's Disease

Dexterity evaluation is pivotal in assessment of hand function during rehabilitation of people with Parkinson's disease. Scarce information on hand dexterity in Parkinson's disease (PD) using standard tools, prompted the present study, aiming to evaluate fine and manual hand dexterity using the Minnesota Manual Dexterity Test (MMDT) and the Nine-Hole Peg Test (NHPT). The secondary aim was to explore the association of fine and manual dexterity with motor function as measured by the Movement Disorder Society Unified Parkinson's Disease Rating Scale (MDSUPDRS). Fifty-four people with PD and 54 healthy ageand gender-matched adults were studied using a standard protocol. The average time taken by healthy people to perform the manual dexterity test was 66.6 ± 7.09 s; for people with PD, the time taken was 116.8 ± 45 s, which was highly significant (p < 0.0001). The average time taken by healthy people to perform the fine dexterity test was 26.1 ± 4.66 s (left hand) and 26.3 ± 14.18 s (right hand); for people with PD, the time taken was 39.5 ± 14.31 s (left hand) and 36 ± 12.22 s (right hand). People with PD demonstrated significantly low manual dexterity (p < 0.0001) and fine dexterity compared with healthy people (p < 0.0001). People with PD were less dexterous in unimanual tasks compared with bimanual tasks (p < 0.0001). No significant difference was noted in fine dexterity between the right and left hands. A moderate positive correlation (r = 0.63, p < 0.0001) was noted between manual dexterity and motor function on the MDSUPDRS; a low positive correlation (r = 0.44, p < 0.0001) was noted between fine dexterity and motor function. Findings from the present study will provide clinicians with reference values for manual and fine dexterity training in the hand rehabilitation of people with PD.
