What is the World Recommending? An International Perspective of Isometric Handgrip Training for Lowering Blood Pressure

Hypertension (chronically elevated blood pressure) is the leading risk factor for death and disability worldwide. An effective exercise intervention to prevent and treat this condition is isometric handgrip training, a form of isometric resistance exercise. Little is known about the global endorsement of this novel intervention among international hypertension management guidelines. Therefore, the overarching exploratory objective of this study was to locate, compile, and discuss international recommendations for this form of exercise within hypertension management guidelines. A final total of 23 original English-language hypertension management guideline editions published between 2015 and June 2020 were included in the discussion. A total of five governing bodies had a formal recommendation for isometric resistance exercise, and of these, only three specifically mentioned isometric handgrip training. Moreover, five included related statements and/or referenced evidence from reputable isometric resistance exercise literature. In contrast, four governing bodies advised against isometric resistance exercise due to safety concerns. It is critical to determine the awareness levels, as well as standards for recommending isometric resistance exercise, in order to navigate the next steps and encourage maximal uptake of nonpharmaceutical hypertension interventions on a global scale.
